Born December 27, 1910 in Pittsburgh, Allegheny County, Pennsylvania. He was a children of Martin Cervenak and his wife Mary Kachurik.

Description

John, who went by the name Jack, was of average build and stature (5 foot 7 inch). He had a light complexion along with brown hair and brown eyes.
